My name is Dr. Craig Singer and I have been a Chiropractor for twenty years. Through my professional training and personal experience in practice, there are things that I know work well. When dealing with acute injuries we are taught and then practice the basic philosophy of cooling down an area to reduce, reverse and prevent the inflammatory process. That has not changed and will not in the foreseeable future. When an injury is diagnosed (history, inspection, palpation, range of motion, orthopedic, neurologic, x-ray, and lab) you set forth a plan. Once a cooling protocol is set in motion, how you implement it can effect your patient’s outcome.
I am a great proponent and advocate of cooling/icing. I ice everything that is acute and preach home icing to my patients from day one. I even hand out home care, personal ice packs as they leave the office. Now as any health care provider knows, there are risks involved that must be discussed with patients and icing is no exception. You can get minor frostbite in some patients who don’t listen and frostbite is hard to explain in the state of Florida (especially to your malpractice carrier). Worse yet, extreme vasoconstriction of vessels during icing can damage tissue, further delaying healing. There are also patients who won’t ice, are generally non compliant and do not follow home care instructions very well. Non-compliance is a big determining factor in overall patient outcomes and recovery. As Doctors we can only manage our plans and hope for the best.
